Community Based Day Services
Day services at Opportunities Unlimited are community based
i.e. individuals spend their day involved in activities of
interest that facilitates their interaction and involvement
with members of the community. Some places where individuals
spend their time include – The RI Community Food Bank,
Cranston Senior Center, Meals on Wheels, RI Hospital, SPCA.
Staff assistance and support is available to insure a
successful experience.
Supported Employment
ORS certified vendor in providing job seeking and job
coaching services. Inclusive community employment focus for
individuals seeking paid employment within their community.
Family Support
Services and supports are provided to an individual living
at home with family. These supports/services can be designed
to “teach” skills such as household management and budgeting
and/or can be community based. An example of community based
supports/services would be accessing recreational activities
of interest.
Supported Living
Individuals who live on their own or with a roommate and do
not require an intense level of staff support are eligible
to receive supported living services. Staff supports are
designed around the specific needs of the individual and can
range from 2-3 hours a week up to 40-50 hours a week.
Services are designed to support the individual to maintain
the level of independence needed to live in the community.
24 hour Supported Living Services
Individuals who require a more intense level of support and
supervision may choose to live in one of the 6 homes or 5
apartments operated by this agency within which 24 hour
staff supports are available. Focus within these homes is on
supporting individuals to develop skills needed to live,
work and play successfully in their community. A variety of
clinical supports usually are available within these homes.
Clinical services
A variety of clinical services are available. Examples of
clinical services available include nursing, psychology,
nutrition, counseling. Other clinical services such as
physical therapy, occupational therapy and speech therapy
that may be required by an individual are accessed as
needed. On call nursing supports are also available to those
that require this level of nursing service.
Case Management
Individuals needing assistance with obtaining affordable
housing, a roommate, medical services, transportation and
other services can access the skills of management staff at
the agency.
Transportation
As part of the individual Support Agreement, assistance with
transportation may be identified as a need. This assistance
may include teaching skills needed to access public
transportation as well as provision of transportation to and
from medical appointments, marketing, banking, recreation
events, and to/from work.
The scope of supports and
services in any of the areas is requested by the individual
and detailed within the support agreement.
